CVS Caremark will be managing your prescription drug benefit plan on behalf of Verizon starting January 1, 2025 . You will have continued access to affordable medication and convenient options for filling prescriptions, including setting up home delivery via the Caremark App.

You will be receiving more information closer to January 1, 2025, including:

• How to register and access your Caremark.com account. On or after December 9, 2024, you'll be able
to register and see plan information, check medication costs, coverage, pharmacy network status, find
savings opportunities and more.
• Prescription ID cards which will be mailed to your household. ID cards will also be available to view and
print on the Caremark website. A digital ID card can be downloaded after January 1, 2025.
• Physician approval processes in the event a medication review is necessary. We'll let you know exactly
what you need to do.
• If you take a specialty medication (for a condition like rheumatoid arthritis or multiple sclerosis), you’ll
receive information on how to choose a select participating pharmacy.
