In order to receive the $600 credit for non-tobacco users you must certify that you and your dependents are not tobacco users, or certify that you will complete a Tobacco Cessation Program. Otherwise the company will default you into the tobacco user category and you will not get the $600 credit.
This is a different process than filling out the Health Assessment questionnaire. They are two separate processes, both accessed through Verizon Benefits Connection - one says "Enroll in my 2013 Benefits" (which includes certification of non-tobacco use) and one says "Health Assessment."
When you fill out the Health Assessment questionnaire you get a $100 credit. The questionnaire asks if you are a tobacco user. BUT THIS DOES NOT APPLY THE $600 CREDIT FOR NON-TOBACCO USERS TO YOUR ACCOUNT. To get the $600 credit you must re-enroll in your benefits and certify neither you nor your dependents are tobacco users, or that you will complete a Tobacco Cessation Program. You will then see the $600 credit applied to your total premium costs. You can do this on-line at Verizon Benefits Connection or by calling 1-877-489-2367. Open enrollment is extended until December 6th. Members have until December 31, 2012 to complete the Health Assessment. The Health Assessment cannot be completed by phone, it must be done online.
If you don't do anything, you will be defaulted to your current plan, AND you will default into the "tobacco user" category and will not receive the $600 credit. Again, to receive the credit you must call or go on-line and certify you are not a tobacco user.
Important Reminder to those who do not have Internet Access or prefer to call the Benefit Center: If members need any printed materials mailed to your home, call the Verizon Benefits Center at 1-855-4VzBens (1-855-489-2367) as soon as possible.
